32-Check Health Monitor + Stale Process Cleanup
Expanded the system health monitor from 28 to 32 checks — now covers entity graph integrity, memory embedding gaps, session health, and storage growth. Built a separate stale process scanner that identifies zombie sessions consuming memory and alerts via Discord, without ever auto-killing anything.
