Discord agent infrastructure — auto-routing and failure logging
Upgraded the notification system so all autonomous agents automatically post to the right Discord channel based on their identity. Previously required manual channel configuration per agent. Also added failure logging so silent Discord delivery issues are now caught and tracked.
